Release of Liability and Waiver Form *

1. Agreement to Participate I hereby acknowledge and agree that I am voluntarily participating in Pilates classes and/or related activities offered by Ketchikan Pilates (hereinafter referred to as "KP") located in Ketchikan, Alaska. I understand and agree that participation in Pilates involves inherent risks, including but not limited to, physical injury. 2. Assumption of Risk I acknowledge that I am aware of the risks associated with physical exercise, including the risk of injury. I understand that it is my responsibility to consult with a healthcare professional prior to engaging in any physical activity, including Pilates. I voluntarily assume all risks of injury, illness, or death that may result from my participation in the KP’s classes or activities. 3. Release of Liability In consideration of being permitted to participate in the KP’s classes and activities, I, for myself, my heirs, executors, administrators, and assigns, hereby release, waive, discharge, and covenant not to sue Ketchikan Pilates, its owners, instructors, employees, agents, and affiliates from any and all claims, demands, actions, or causes of action arising out of or related to any loss, damage, or injury, including death, that may be sustained by me or to any property belonging to me, whether caused by the negligence of the KP or otherwise. 4. Medical Conditions I represent that I am in good health and have no physical conditions or impairments that would prevent me from safely participating in the Pilates classes. I acknowledge that it is my responsibility to inform the instructor of any medical conditions or injuries that may affect my participation. 5. Indemnification I agree to indemnify and hold harmless Ketchikan Pilates from any and all claims, liabilities, damages, and costs, including reasonable attorney’s fees, arising out of or related to my participation in the KP’s classes and activities. 6. Governing Law This Release of Liability shall be governed by and construed in accordance with the laws of the State of Alaska. Any disputes arising under or in connection with this Release of Liability shall be subject to the exclusive jurisdiction of the courts located in Ketchikan, Alaska. 7. Acknowledgment of Understanding I have read this Release of Liability and Waiver Form in its entirety. I fully understand its contents and am aware that it is a release of liability and waiver of legal rights.

Photography and Video Release Waiver *

This Photography and Video Release Waiver (the "Release") is entered into on [Date], by and between [Name of Photographer/Organization] (the "Photographer") and [Participant's Full Name] (the "Participant"). 1. Permission to Use Media The Participant hereby grants the Photographer and its representatives, employees, and agents the right to take photographs and/or video recordings of the Participant during classes or events at Ketchikan Pilates. The Participant also grants permission for these photographs and videos to be used in promotional materials, educational content, websites, social media, or any other form of media for commercial or non-commercial purposes. 2. Purpose The photographs and videos may be used for, but are not limited to, the following purposes: • Marketing and promotional materials • Website and social media content • Educational and training materials 3. Compensation The Participant acknowledges and agrees that they will not receive any compensation or payment for the use of their images or videos, unless otherwise agreed upon in writing. 4. Duration and Scope The rights granted by this Release are perpetual and worldwide. The Photographer may use the media indefinitely and may transfer these rights to other entities or individuals as needed. 5. Revocation The Participant understands that this Release cannot be revoked once the media has been used or published. If the Participant wishes to withdraw consent in the future, they must contact the Photographer in writing, but this will not affect any media already published or distributed. 6. Release of Claims The Participant agrees to release, discharge, and hold harmless the Photographer and its representatives from any and all claims, demands, and liabilities arising out of or in connection with the use of the photographs or videos, including any claims for defamation or invasion of privacy. 7. Governing Law This Release shall be governed by and construed in accordance with the laws of the State of Alaska. 8. Agreement By signing this Release, the Participant acknowledges that they have read and understood its contents and voluntarily agree to its terms.
